Choosing hospital management software (HMS) is less about features on a brochure and more about one question: will every charge, result and bed movement be recorded once, at the moment it happens? If the answer is yes, discharges get faster, revenue stops leaking and staff stop chasing paper. If not, you have added another system to the pile.
This checklist is for hospitals, nursing homes and multi-speciality clinics in India. You can follow along with our hospital management software demo, which prepares a discharge bill from charges posted during a stay.
1. Start from your departments, not the software
List the departments you run today — OPD, IPD, pharmacy, laboratory, radiology, operation theatre, billing, insurance desk — and the ones you plan within two years. Then list who does what in each. Software should fit this map; if a vendor's answer to every question is "you'll change your process", be careful.
2. One account per patient stay
The heart of an HMS is the patient account. Room nights, pharmacy issues, lab tests, procedures and doctor visits should post to the stay as they happen, from the department that provides them. Ask to see a discharge bill produced from a test admission. If billing still has to collect slips, the system isn't doing its job.
3. Insurance and TPA billing
For most hospitals, insurance decides cash flow. Check that the software handles:
- Package rates and what they include.
- Pre-authorisation requests, approvals and enhancements.
- The patient's share worked out per admission.
- Claim documents generated from the same records as the bill.
4. Lab and radiology connections
Retyping results is slow and risky. Ask whether your analysers can send results directly — most support standard interfaces such as HL7 or ASTM — and how radiology images and reports are linked to the patient. A separate pathology lab system may be worth integrating if your lab is large.
5. ABDM and digital health records
The Ayushman Bharat Digital Mission lets patients link records to their ABHA number. If digital health records matter to your patients or your schemes, check whether the HMS can integrate with ABDM and what certification is involved.
6. OPD and the waiting room
A crowded waiting room is the first thing patients notice. Look for token queues, estimated waiting times and appointment booking — or connect a clinic appointment and live queue app so patients can wait at home.
7. Hosting, security and patient data
Health data is sensitive personal data under the Digital Personal Data Protection Act. Decide:
- Where it runs — on your own server, in your cloud account or the vendor's. Many hospitals prefer on-premise for control and local-network speed.
- Who sees what — access by role, so a billing clerk doesn't read clinical notes.
- What is logged — every view and change, kept for audits.
- Backups and recovery — how often, where, and how long a restore takes.
8. People on the floor
Nurses, ward boys and front-desk staff will use the system most. Screens must be fast, simple and, where helpful, in Hindi. Doctors increasingly want their OPD list, results and prescriptions on a phone. Ask for a hands-on trial with your own staff, not just a demo by the vendor.
9. Roll it out in stages
- Registration, OPD and billing first — they touch every patient.
- IPD and the bed board next, with a short parallel run of the old registers.
- Pharmacy and lab, connecting the analysers.
- Insurance desk and reports once the data flowing in is reliable.
Go live department by department, on weekdays, with the vendor's team on site for the first days.
10. Measure before and after
Pick four numbers and record them for a month before go-live: minutes from discharge order to final bill, charges missed at audit, bed turnaround time and TPA queries per month. Compare them three months later. That is how you know the software is earning its place.
Ready-made or built around you?
Ready-made HMS products suit hospitals whose workflows match the product. A system built around your hospital suits groups with unusual departments, packages, approval chains or integrations — and you own it rather than paying per bed every year. Read more on our healthcare industry page.
